Program Guidelines
In order to receive supplies you must follow the following criteria
below:

1- Complete registration form.

Mail or fax your application using the information below.

3-
Once we receive your application we will notify you by mail or
telephone of your pick up location and time.
Please arrive on
time for your scheduled appointment and bring all children to
appointment. If you are unable to keep your appointment please
cancel within 24 hours.

4-Each child in your family is eligible to receive (1) backpack

(based on availability)
and school supplies once per year.
However, foster parents/caregivers are eligible to receive back
packs for new children that may enter into the home throughout
the school year. If child can not be present at time of pickup you
must present child’s birth certificate, medicaid or social security
card for verification.

5-You cannot request supplies for someone else's child(ren)
unless you are a foster parent or legal guardian of the child(ren).

6-If you are applying for school supplies for your child(ren) and
only speak spanish you will need to bring a translator with you
when you pick up supplies.

7-This must be the only program in which you have or will
request assistance with a back pack and school supplies for your
children.

Please note: This program is only intended to supplement your
child with some of the school supplies they need. We ask that
you only request supplies if your child(ren) are truly in need of
them. Children must be in grades K-12 to receive supplies.

Please Note: We are currently unable to deliver supplies to
applicants because of the lack of funding for this program.

The availability of school supplies and backpacks are based on
the amount of community donations that we receive. Donations
to help this organization are needed and always welcome!
